Presentation

Presented with worsening neck swelling and dysphagia.

Patient Data

Age: 80 years
Gender: Female

Chest radiograph

x-ray

Large neck mass causing tracheal deviation towards the left. 

Clear lungs. 

Normal heart size. 

Case Discussion

Goiters can present with dysphagia, hoarseness of voice or breathing difficulties.

The commonest cause for these appearances is a thyroid goiter.   The first things to do are:

-  see if there are older films showing the same (and therefore benign)

-  see if an ultrasound of the neck has even been undertaken

-  see if the patient has had a CT chest for another indication and the goiter is evident

If any of the above applies there is no need to cause undue alarm about reporting a mediastinal mass which will immediately be assumed to be a malignancy until proven otherwise.
