Presentation

Known case of chronic liver disease.

Patient Data

Age: 60 years
Gender: Male

CECT abdomen (Triple phase)

The liver has irregular margins, the portal vein is dilated and some collateral vessels are seen in the upper abdomen inferring chronic liver disease with features of portal hypertension.

Arterial and early portal venous phase enhancing lesion in the segment VI/VII of the liver with washout in the delayed phase, likely hepatocellular carcinoma in the background of chronic liver disease.

Another small peripherally calcified non-enhancing lesion in the segment VIII of the liver, likely a benign lesion. In addition, a small cyst is noted in the segment VIII of the liver.

Large calculus in the gallbladder lumen, suggestive of cholelithiasis.

Case Discussion

The arterial and early portal venous phase enhancing lesion in the liver with washout in the delayed phase in the background of chronic liver disease is highly suggestive of hepatocellular carcinoma unless proven otherwise.
