Horseshoe kidney with circumaortic left renal vein

Case contributed by Octavio Guardado López
Diagnosis almost certain

Presentation

Patient was schedule for follow up of renal lithiasis

Patient Data

Age: 50 years
Gender: Male

Imaging study was reported with horseshoe kidneys by fusion of inferior poles associated with bilateral renal lithiasis and circumaortic left renal vein as an anatomical variant.

Case Discussion

In this case two anatomical variants are found, a circumaortic left renal vein and a horseshoe kidney with fusion points in lower poles, both uncommon in frequency and important to report since treatment planning is not the same as in the general population, particularly in this patient with nephrolithiasis, in which common options like shockwave lithotripsy, percutaneous surgery or ureteroscopy protocols must be "adjusted" for an abnormal kidney, with extra care for the vascular variant and possible associated complications.
