Recurrent vomiting after every feed and crying.

A case of hypertrophic pyloric stenosis where the pylorus muscle appears thick. The muscle thickness is approximately 5.3 mm and 4.9 mm, with a length of the pylorus around 17.2 mm and a diameter of approximately 15.6 mm.

Case Discussion

Clinically, the infant should have recurrent episodes of vomiting, failure to thrive, hypochloremic, and hypokalemic metabolic acidosis. Ultrasound criteria are pylorus muscle thickness > 3 mm, diameter > 15.0 mm, and length > 15-17 mm. Dynamic studies should be performed during and after feeding to observe the opening of the pylorus and the movement of gastric contents into the duodenum.
