Diagnosis probable

Presentation

Right orbital swelling and pain.

Patient Data

Age: 65 years
Gender: Female

Diffuse swelling and enhancement of the right inferior oblique and inferior rectus muscles.

Bilateral scleral calcifications are noted.

Mild right proptosis.

Diffuse swelling and enhancement of the right inferior oblique and inferior rectus muscles untill their insertions. Mild stranding of retrobulbar fat is noted.

Mild right proptosis.

Case Discussion

Features are suggestive of a myositic orbital pseudotumor. It is an idiopathic inflammatory disease of the orbit most commonly involving the extra-ocular muscles with the involvement of tendinous insertions, which differentiates it from thyroid ophthalmopathy.
