Presentation

A ballet dancer with acute onset of right iliac crest pain after landing on his pronated right leg.

Patient Data

Age: 15 years
Gender: Male
mri

There is bilateral bone marrow edema at the iliac crests, more prominent on the right.

The iliac crests demontrate irregular borders.

Soft tissue edema anteriorly to the right anterior superior iliac spine.

There are osteophytes surrounding the iliac crests.

Also note highly developed muscles, especially the iliopsoas and adductor femoris.

Case Discussion

Iliac crest apophysitis should be differentiated from avascular necrosis (also known as a Mouchet-Sorrel-Stefani syndrome) because inflammation is the main pathogenetic mechanism.
