Lateral malleolus fracture: value of orthogonal view

Case contributed by Ian Bickle
Diagnosis certain

Presentation

Fall. Ankle pain. Fracture?

Patient Data

Age: 25
Gender: Male

Undisplaced oblique fracture of the lateral malleolus.   Associated soft tissue swelling.

The fracture is not evident on the AP view.

Case Discussion

Orthogonal views are essential for peripheral plain film radiography.  As is demonstrated in this case a single view can be inadequate in identifying pathology.

The fracture is clearly evident on the lateral, but not well seen on the AP view.
