Presentation

Marked knee pain after direct trauma.

Patient Data

Age: 20 years
Gender: Male
mri

Regional bone marrow contusion related to the lateral femoral condyle and anteromedial aspect of the patella elicited high signals in the T2 and STIR WIs. This pattern of marrow edema is characteristic of lateral patellofemoral dislocation pattern and subsequent impaction of the patella against the medial femoral condyle. The patella is currently subluxated in relation to the femoral sulcus associated with the heterogenous amorphous appearance of the medial retinaculum showing interruption of its patellar attachment.

Moderate amount of joint effusion related to the knee joint proper and expanding all synovial recesses.

Associated fine supra patellar synovial plica.

Case Discussion

Findings are most consistent with acute lateral patellar dislocation with medial patellar retinaculum tear as described above.
