Lisfranc injury (weightbearing x-rays)

Medially displaced fracture of the medial base of the right 2nd metatarsal, in keeping with a Lisfranc ligament avulsion fracture. Asymmetric widening between the right 1st and 2nd metatarsal bases.
Otherwise normal bony alignment.
Old healed fracture of the left 5th metatarsal. No other bony abnormality.
Case Discussion
Typical features of an avulsion fracture at the main insertion of the Lisfranc ligament. The degree of malalignment is somewhat subtle but can be typical for these injuries. Weight-bearing x-rays are an alternative to MRI to assess the integrity of the Lisfranc joint.
