Presentation

Patient presenting after slip and fall injury, complaining of severe ankle and lower leg pain. On examination, there is marked deformity at the ankle joint and associated swelling.

Age: 30 years
Gender: Male

This is a classic case of higher degree of injury associated with Maisonneuve fracture.

In this case, there is a comminuted fracture of the medial malleolus, distraction of distal tibio-fibular joint (with associated ligamentous injury), a fracture of talus and a high fibular fracture.

Case Discussion

Per literature, in Maisonneuve fracture, the most common inciting event is an injury that causes forceful external rotation and extroversion at the ankle joint.

The initial injury is often fracture of medial malleolus of the ankle followed by rupture of anterior syndesmosis and then high fibular fracture. There is significant distraction at the distal tibio-fibular joint space.

Further evaluation with additional cross-sectional imaging is recommended to assess the degree of ligamentous injury and any additional associated fractures of tarsals.
