Multicystic encephalomalacia

Presentation

Delayed milestones with microcephaly.

Patient Data

Age: 5 months
Gender: Male
mri

Decreased volume of the cerebral hemispheres with numerous cystic cavities of various size and shape within the cortex and white matter mainly of the frontal and temporal lobes, eliciting a low signal on T1 and FLAIR, high signal on T2 with no restricted diffusion. Ex vacuo dilatation of the lateral ventricles with intact ventricular walls. Atrophy of the corpus callosum. Preserved posterior fossa.

Right parietal subdural hematoma of high signal intensity on both T1 and T2 with internal septation of low signal on T2 and FLAIR sequences, indicating a late subacute subdural hematoma (methemoglobin free). Small left parietal chronic subdural hematoma is also noted.

Small subgaleal occipital hematoma is noted.

Case Discussion

MRI features of a multicystic encephalomalacia representing the result of an extensive brain insult in term neonate from ischemia, infection or developmental insults in the antenatal or early postnatal period. The subdural and subgaleal hematomas are discovered incidentally.
