Multicystic encephalomalacia

Diagnosis probable

Presentation

According to the patient's parents, he has had neck flaccidity along with limb stiffness since childhood. No history of birth trauma is narrated.

Patient Data

Age: 10 months
Gender: Male

There are multicystic encephalomalacic areas in bilateral cerebral hemispheres, more marked in bilateral frontal and parietal regions. The temporal poles are spared.

No intra-cranial bleed and calcification is seen.

Early closure of bi-coronal sutures is noted.

The basal ganglia and posterior fossa are spared.

Case Discussion

CT findings are of bilateral and symmetrical multicystic encephalomalacia.

There is associated microcephaly but no intracranial calcification. The temporal poles are relatively spared.

This has a broad differential. Prior ischemia and fetal/neonatal infection are most likely although there was no perinatal or postnatal history to suggest ischemic injury.
