Oligodendroglioma

Presentation

Seizures, headaches and features of raised intracranial pressure for 4 months. No fever or travel history

Patient Data

Age: 50 years
Gender: Female

Right frontal ill-defined hypodense mass with ribbon-like calcification, minimal post-contrast enhancement and surrounding cerebral edema. Associated subfalcine herniation, midline shift and contralateral entrapment hydrocephalus.

Right frontal intra-axial mass demonstrating predominantly T1W hypointense, T2W hyperintense signal and serpiginous intra-tumoral enhancement. Subtle focal areas of diffusion restriction on the right posterolateral margins. Blooming artifacts on SWI corresponding to areas of mineralization.  Extensive vasogenic edema extends across the corpus callosum with local mass effect, midline shift together with subfalcine herniation and contralateral entrapment hydrocephalus.

Case Discussion

The patient subsequently underwent surgical debulking with histological confirmation of oligodendroglioma NOS (no IDH status or 1p19q codeletion status is available). 

The differential for calcified glial tumors includes supratentorial ependymoma and rarely glioblastoma.
