Presentation

Acute pelvic pain.

Patient Data

Age: 12 years
Gender: Female
mri

The left ovary is significantly enlarged (8 x 5 x 4 cm) showing a heterogeneous texture with a peripheral T2 hypointense zone showing multiple small follicles. The ovarian pedicle is thickened and twisted.

The right ovary is prominent in size (3 x 3.4 x 4.3 cm) with multiple small peripheral follicles.

The uterus is unremarkable for the patient's age.

There is no free fluid in the cul-de-sac of Douglas.

The features are consistent with left ovarian torsion.
