Papillary fibroelastoma - mitral valve

Presentation

Routine medical check up. No signs of infection.

Patient Data

Age: 70 years
Gender: Male
ultrasound

Both mitral valve leaflets have small, pedunculated, round, echo-dense masses attached to the atrial side. The mass on the anterior mitral valve leaflet measures 8.5 mm, while the one on the posterior mitral valve leaflet measures 10 mm. The masses are causing mitral inflow obstruction by Doppler-flow interrogation and moderate to severe eccentric mitral regurgitation.

Normal aortic valve. Normal ventricles with good biventricular systolic function.

Case Discussion

Papillary fibroelastomas (PFEs) are benign cardiac tumors developing mostly from the cardiac valves. PFEs have also been associated with valve malfunction, which can result in obstructions, regurgitation, or a combination of the two. Though benign and often asymptomatic, PFEs can cause potentially serious complications such as embolic phenomena which include stroke or transient ischemic attack or angina, acute coronary syndrome, and myocardial infarction.

Differential diagnosis can be Lambl’s excrescences, or thrombus.
