Presentation

Swelling of the parotid glands in an obese diabetic patient with hypertriglyceridemia.

Patient Data

Age: 50 years
Gender: Female
mri

Both parotid glands are diffusely enlarged of high signal on T1 and T2, attenuated on T1and T2 fat sat sequences with no intraglandular focal lesion or cervical lymphadenopathy.

Normal appearance of the submandibular glands.

Case Discussion

Enlarged parotid glands with diffuse fatty replacement in an obese diabetic patient with hypertriglyceridemia are highly suggestive of parotid liposubstitution.

Parotid liposubstitution is physiological with age but may be associated with local or systemic conditions such as hypertriglyceridemia, adnexectomy, Sjögren's syndrome, human immunodeficiency virus, alcoholics, diabetics, lymphoma and sarcoidosis.
