Presentation

Known for ankylosing spondylitis (no report available), presented with a lower abdominal discomfort. had surgery for appendiceal peritonitis 15 years ago.

Patient Data

Age: 35 years
Gender: Female

A large pelvic peritoneal cystic structure surrounding the left ovary of low signal on T1, high signal on T2 with no restricted diffusion. Mild peripheral enhancement seen on post-contrast sequences.

Bilateral sacroiliitis with involvement of both hip joints.

MRI features most consistent with a peritoneal inclusion cyst in a patient with a history of surgery for appendiceal peritonitis 15 years ago.

Peritoneal inclusion cyst is defined as a cyst-like structure that appears in relation to the peritoneal surfaces, caused by the accumulation of ovarian fluid that is contained by a peritoneal adhesion. It occurs almost exclusively in premenopausal women with a history of previous abdominal or pelvic surgery, trauma, pelvic inflammatory disease, or endometriosis.
