Retained pacemaker wire

Case contributed by Ian Bickle
Diagnosis almost certain

Presentation

Shortness of breath. CCF?

Patient Data

Age: 95 years
Gender: Male
x-ray

Right sided dual lead pacemaker.

Long length of pacing wire, not connected to the current pacemaker box, projected over the left atrium.

Lungs clear.

Case Discussion

The right side is not typically used for initial pacemaker insertion.

This combined with the third wire on the radiograph, not connected to the pacemaker unit is consistent with this being a retained wire.

It was present in multiple prior films dating back several years.
