Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Pelvic pain.

Patient Data

Age: 60 years
Gender: Female

A large well-defined thin-walled right ovarian unilocular cyst (6 cm) with a homogeneous fluid content of a low signal on T1 and high signal on T2 with no restricted diffusion or enhancement on postcontrast sequences. No solid component or internal septation is seen.

Case Discussion

The MRI features of a unilocular ovarian cyst of homogeneous fluid content with no restricted diffusion, internal septation or solid component in a postmenopausal woman are highly suggestive of a serous inclusion cyst.

In such case ( ovarian cyst >5 cm), the recommendation is follow-up either in 3-6 months for resolution/re-characterization or in 6-12 months for growth rate assessment.
