Sesamoid fracture

Diagnosis almost certain

Presentation

Pain along the plantar surface of the first metatarsophalangeal joint after foot trauma for about two months.

Patient Data

Age: 30 years
Gender: Female
mri

Left foot, fibular (lateral) sesamoid of the hallux has high signal intensity consisting with edema which is best seen on coronal PD fat-sat images and also lost its normal fatty bone marrow signal on T1 images. The fracture line which has a low signal is best seen on sagittal fat-sat T2 images. There is an increased amount of fluid in the first metatarsophalangeal joint.

x-ray

In these two planes, fissure line of fracture is seen on fibular (lateral) sesamoid of the hallux.

Case Discussion

This part of fracture must be differentiated from bipartite sesamoid, MR imaging is a useful tool for diagnosing fracture and also additional conditions.
