Presentation

Abdominal distension and constipation followed by diarrhea.

Patient Data

Age: 75 years
Gender: Male

The rectum is collapsed and the sigmoid colon appears markedly distended measuring about 13 cm in maximum diameter with twisting of the mesenteric vessels and a transitional zone at the level of the rectosigmoid junction, suggesting sigmoid volvulus.

Case Discussion

The sigmoid colon twisting on the sigmoid mesocolon causing obstruction.

CT features:

  • large gas-filled loop lacking haustra, forming a closed-loop obstruction

  • whirl sign: twisting of the mesentery and mesenteric vessels

  • bird beak sign: if rectal contrast has been administered

  • x-marks-the-spot sign: crossing loops of bowel at the site of the transition

  • split wall sign: mesenteric fat seen indenting or invaginating the wall of the bowel

  • steel pan sign: a close resemblance to the percussion instrument known as a steelpan
