Sinding-Larsen-Johansson disease

Diagnosis almost certain

Presentation

Chronic knee pain, football player.

Patient Data

Age: 11 years
Gender: Male

Mild tendinosis of the patellar tendon proximal attachment evidenced by mild focal thickening and intrasubstance increased signal, yet no evidence of fibers disruption.  Associated marrow edema of the related lower patellar pole with soft tissue edema of the adjacent Hoffa’s fat pad. Features are suggestive of Sinding-Larsen-Johansson syndrome.

Small subcortical patchy marrow edema is seen involving the medial femoral epiphysis

Minimal knee joint effusion

Case Discussion

MRI findings are suggestive of Sinding-Larsen-Johansson disease a pediatric version of "jumper's knee".
