Slow flow vascular malformation - cervicofacial

Presentation

Painless right cervicofacial swelling, progressively increasing in size since birth.

Patient Data

Age: 7 years
Gender: Female

There is a large lobulated right cervicofacial soft tissue mass composed of numerous serpiginous vascular structures, centered on the parotid gland, eliciting a low signal on T1, high signal on T2 with with no restricted diffusion. The post-contrast dynamic sequences show a progressive enhancement with almost fill-in on delayed sequences. It shows an extension to the following spaces:

  • and parapharyngeal space
  • sub-mandibular space with involvement of the submandibular gland, and premandibular space, displacing the masseter muscle
  • sublingual space with involvement of the ipsilateral muscles of the floor of the mouth and the tongue
  • carotid space encasing the internal carotid artery along the carotid canal up to the cavernous sinus
  • infratemporal and pterygopalatine fossae
  • along the SCM muscle
