Presentation
Painless right cervicofacial swelling, progressively increasing in size since birth.
Patient Data
There is a large lobulated right cervicofacial soft tissue mass composed of numerous serpiginous vascular structures, centered on the parotid gland, eliciting a low signal on T1, high signal on T2 with with no restricted diffusion. The post-contrast dynamic sequences show a progressive enhancement with almost fill-in on delayed sequences. It shows an extension to the following spaces:
- and parapharyngeal space
- sub-mandibular space with involvement of the submandibular gland, and premandibular space, displacing the masseter muscle
- sublingual space with involvement of the ipsilateral muscles of the floor of the mouth and the tongue
- carotid space encasing the internal carotid artery along the carotid canal up to the cavernous sinus
- infratemporal and pterygopalatine fossae
- along the SCM muscle
Case Discussion
MRI features of a large cervicofacial mass most consistent with a slow-flow vascular malformation centered on the parotid gland with multidirectional extension.
