Subcapsular perirenal hematoma

Presentation

Sudden onset abdominal pain. No trauma.

Patient Data

Age: 75 years
Gender: Male

Subcapsular left perinephric hematoma. At the left upper pole is the impression of a 2.8 cm hyperdense renal mass with a cortical defect. Delayed left nephrogram with no excretion of contrast on the 2 min delay. 1.5 cm left upper pole simple renal cyst.

Right kidney has a normal appearance with non-specific perinephric fat stranding.

Right inguinal hernia containing non-obstructed cecum. 

Case Discussion

Spontaneous subcapsular perirenal hematomas do occur, however, an underlying lesion should not be discounted and follow-up imaging may be of benefit if there is an unclear cause.
