Bilateral ocular proptosis.
Both inferior rectus muscles are thickened significantly with tendon insertion sparing. Other extraocular muscles are also show mild thickening without tendon insertion involvement.

Typical appearances of thyroid associated orbitopathy. The inferior rectus muscle is usually the first and most severely affected muscle. The order of involvement can be remembered using the mnemonic I'M SLOW.
