Presentation

Headaches with vertigo.

Patient Data

Age: 85 years
Gender: Male
mri

Well-defined soft tissue mass enlarging the left foramen ovale and extending to the infratemporal fossa passing between the tensor veli palatini and lateral pterygoid muscle. It elicits a low signal on T1, and a high signal on T2 with a vivid homogeneous enhancement on postcontrast sequences. No calcification or hemorrhagic component is seen on the GE sequence.

Cerebral volume loss with small vessel ischemic change is noted.

MRI features of an enhancing soft tissue mass enlarging the foramen ovale and extending to the infratemporal fossa, suggestive of a trigeminal schwannoma of the mandibular diversion (V3), incidental finding in this case.
