Urethral diverticulum

Presentation

Chronic dribbling of urine with recurrent urinary tract infection (UTI) and a palpable swelling at the posterior aspect of the penile base

Patient Data

Age: 15 years
Gender: Male
ultrasound

Both testes and epididymides have normal appearance. No scrotal masses or hydrocele.

A sac like fluid-filled protrusion with subtle debris is noted at the posterior aspect of the penile base. The sac appears to have a blind ended tract probably leading into the urethra.

Micturating cystourethrography (MCU) was recommended for further assessment.

x-ray

Water soluble contrast was introduced into the urinary bladder via a syringe inserted into the distal urethra.

Diverticula are noted in the urinary bladder.

Focal outpouching of the bulbopenile urethra is demonstrated.

Post micturition volume is significant.

Case Discussion

This is a case of urethral diverticulum. Subtle debris noted in the diverticulum sac on ultrasound are suggestive of a urinary tract infection. MCU examination reveals bladder diverticula alongside which further explains the recurrent urinary tract infections as a result of urine stasis.
