Presentation

Right ankle pain, swelling, and tenderness following a fall.

Patient Data

Age: 60 years
Gender: Male
x-ray

Acute minimally displaced spiral fracture of the right distal fibular shaft which appears to involve the tibiofibular syndesmosis. There is mild widening of the medial clear space.

Soft tissue swelling about the ankle joint with suggestion of small joint effusion.

Prominent plantar spurring. Small enthesophyte at the insertion of the Achilles tendon.

Well-corticated ossific density at the lateral base of first distal phalanx likely related to old trauma.

Case Discussion

This represents a Weber B ankle fracture, defined as a simple fracture to the bottom part of the fibula.

The patient was treated non-operatively with a cast.
