Diagnosis almost certain

Presentation

Right knee swelling, tenderness, redness, pain and limited motion.

Patient Data

Age: 1 year
Gender: Male

X-ray of the right knee showed marked joint swelling with effusion.

Marked right knee joint effusion with supra-patellar extension is seen. No diffusion restriction.

Diffusely thickened synovium with edematous changes and diffuse enhancement on contrast sequences. Stranding of Hoffa’s fat pad and irregular Hoffa’s fat pad sign present.

Normal signal of the epiphyseal cartilages of the lower femur and upper ends of the ipsilateral tibia and fibula.

Multiple enlarged reactive looking lymph nodes at popliteal fossa, the largest measuring 9 x 5 mm in dimensions.

Case Discussion

The patient’s clinical presentation lacks the constitutional and local manifestations of septic etiology, these findings represent changes of transient synovitis.

On MRI, there is often signal abnormality in adjacent bone marrow in septic arthritis.
