Ovarian serous cystadenofibroma

Diagnosis almost certain

Presentation

Lower abdominal pain for 10 months, getting worse over the last 2 months. Irregular menstrual history.

Patient Data

Age: 14 years
Gender: Female
ultrasound

Large well-defined anechoic structure measuring approximately 15 x 13 x 8 cm in the midline pelvis. No septations, solid component or internal vascularity is seen. 

Large cystic lesion originating from left ovary measuring 16 x 14 x 7 cm displaying homogeneous low signal intensity on T1 and high signal intensity on T2. A few small peripheral nodules displaying low signal intensity on both T1 and T2 are seen along its right posterior wall. The uterus and the right ovary are normal. 

Case Discussion

Procedure: Laparoscopic left ovarian cystectomy.

Diagnosis: serous cystadenofibroma. Negative for malignancy.
