Paraumbilical hernia - rounded pelvic opacity

Presentation

Back pain.

Patient Data

Age: 60 years
Gender: Female

Diffuse spondylodegenerative changes.

L4-5 facet joint arthropathy with mild degenerative spondylolisthesis of L4 over L5.

Decreased height of L5-S1 disc space with vacuum phenomena.

Mild retrolisthesis of L1 over L2 and L2 over L3.

Rounded homogeneous well-defined opacity over the pelvis, seen on frontal view.

Diffuse spondylodegenerative changes.

Mild spondylolisthesis of L4 over L5 vertebra.

Double-level mild retrolithesis at L1-2 and L2-3 levels.

L1-2 through L5-S1 mild diffuse posterior disc bulges, effacing ventral thecal aspect and mildly encroaching upon the inferior aspect of exit foramina bilaterally.

Lumbar apophyseal arthropathy is noted, notably on L4-5 and L5-S1 levels.

Infraumbilical hernia transmitting omentum is noted. A small supraumbilical hernia is also noted.

X-ray done since 2 years

The rounded opacity was also seen over the pelvis, on frontal view.

Same lumbar spine degenerative changes were noted.

Case Discussion

Abdominal wall hernias can appear as a rounded opacity on plain x-ray. The homogeneous appearance is due to the only omentum content and large size caused it to appear over the pelvic region.
